The premedical questionnaire is to be filled out by students applying to medical school this summer (i.e. for entrance to the class beginning in the fall of 2008).

Following are the instructions for filling out the questionnaire. Please read them carefully.

• First read the questions and then write the essays on your own computer. A given essay question should generally not exceed a page in length.
• Calculate your GPA in the AMCAS format (instructions on the form).
• When you have finished the essays, return to the form and enter in your biographical information and GPA within Netscape or Internet Explorer. Note: use the tab key or the mouse to move between fields. Pressing the enter key in single-line fields will submit the form prematurely.
• Cut and paste the essays into the correct fields within your browser.
• Click on the submit button at the bottom of the questionnaire.
• You will receive email confirmation that we have received your completed form generally within a week.
• This form must be submitted by July 1, 2007.
